Your Rights as a Patient
As a patient, you have the right to:
- Be informed in advance about the care and services you will receive, including any changes to your care plan.
- Receive information, both verbally and in writing, regarding charges, insurance coverage, and any costs for which you may be responsible.
- Receive information regarding the scope of services offered by UroStat Healthcare and any limitations of those services.
- Participate in the development and review of your care or service plan.
- Refuse care or treatment after being informed of the potential consequences of that decision.
- Be informed of your rights under state law regarding Advance Directives, when applicable.
- Be treated with dignity, respect, and consideration at all times.
- Identify UroStat Healthcare personnel through proper identification.
- Be free from mistreatment, neglect, abuse, exploitation, or misappropriation of property.
- Voice concerns, complaints, or grievances regarding care, services, personnel, or policies without fear of discrimination, retaliation, or interruption of service.
- Have complaints investigated and addressed appropriately.
- Receive confidentiality and privacy regarding all personal health information and medical records.
- Be informed of UroStat Healthcare’s policies regarding the disclosure of medical records.
- Choose your healthcare providers, including your attending physician when applicable.
- Receive appropriate care and services without discrimination.
- Be informed of any financial relationship or benefit related to referrals made on your behalf.
- Be informed of your responsibilities as a patient.
Your Responsibilities as a Patient
To help us provide the best possible service, patients are responsible for:
- Completing and submitting forms necessary to receive services.
- Providing accurate medical, insurance, and contact information and notifying us of any changes.
- Participating in the services and care provided.
- Properly maintaining any equipment or supplies provided.
- Notifying UroStat Healthcare of any concerns regarding products, services, or care.
Decision-Making Assistance
If a patient is unable to make medical or other decisions, family members, legal guardians, or other authorized representatives may be consulted to assist with decision-making and care planning.

Patient Acknowledgment
